This service aims is to help adults live longer and healthier lives and be independent for as long as possible. This service can make sure that you have the most appropriate support you need, when you need it, for as long as you need it.

Contact the Adult Access Team if:

• you have a health condition or disability that is making it difficult to manage everyday activities; or
• you provide unpaid care for a family member or friend and need some support; or
• you are experiencing abuse and need help to stop it happening or if you are concerned that someone else is experiencing abuse; or
• you would like to apply for a visual impairment card.
