Dynamic Support Register (DSR) and Keyworker Programme

What is a Dynamic Support Register?

  • Designed to help professionals across organisations, when working with children and young people who have a diagnosis of Learning Disability or Autism.
  • Aims to identify risk.
  • With the view of then mitigating this risk.
  • Identification of provision within community services.
  • Provide alternatives to hospital admission.
  • Support timely discharge for Children and Young People in Hospital.

Why do we need a Dynamic Support Register?

  • Local NHS Services need to understand the needs of Children and Young People  with a Learning Disability & Autism.
  • This ensures that this group of people receive the correct support.
  • The Integrated Care Board (ICB) is required to maintain a register of children and young people with a learning disability or autism who:
    • Display or are likely to display behaviours that will challenge their service provision.
    • Display behaviours or likely to display behaviours where their home life or placement is at risk of breaking down.
    • They have a mental health condition where they are likely to need, or have been admitted to a hospital.

Tees Valley DSR

  • The purpose of the DSR is to ensure that an integrated approach between health, social care and education is in place across Tees Valley, for the risk management of children and young people (CYP) with learning disabilities, autism or other mental health issues.
  • The overall purpose of the group is to ensure robust care packages are in place for hospital discharge, minimise the need for hospital admission, reduce health inequalities and improve outcomes for CYP with learning disabilities, autism or other mental health issues.
  • The group brings together professionals with responsibility for commissioning, procuring and delivering services to individuals who are at risk of admission or who are inpatients in specialist hospitals.
  • The professional who has referred a child or young person into the DSR will provide feedback as to the plan and outcome from the meeting.
  • Advice and referrals to the services that can provide the support you may need will be made by the lead professional
  • Communication will be through the lead professional who referred you to the DSR. 
  • If you referred yourself, the DSR coordinator will inform you of the outcome of the DSR meeting.

Key Worker Programme

What is a Key Worker

  • A person with the right knowledge and influence who will support the young person and their family to navigate Health, Education and Social Care Systems and act as a liaison point for them.
  • In Stockton, Middlesbrough, Darlington, Redcar, Hartlepool and Durham the Key Worker Service is Provided by Daisy Chain

Who is a Key Worker For?

  • A child/young person aged 4-18 years living in the North East & North Cumbria*
  • Diagnosis of autism and/or a learning disability
  • At risk of family breakdown or mental health hospital admission
  • Existing inpatient in a mental health setting
  • Typically on the Dynamic Support Register

What is the Goal of the Key Worker Service

  • Reduce admissions to inpatient care through:
    • Prevent crisis and home and school placement breakdown.
    • Maintain individuals in their own home, and prevent escalation of need where ever possible.
    • Reduce time in inpatient settings.

 What Makes a Good DSR / Key Worker Referral

Current Situation

  • What is currently happening for the Child / Young person
  • Are they at risk of placement breakdown or hospital admission
  • Is the Child / Young person LAC / CIN / CPP
  • Do they have an EHCP and what is the Primary Need
  • What are their preferred pronouns.

Background

  • Who does the child / young person live with?
  • What school do they attend.
  • Who is involved
  • Have they had involvement with police / crisis teams in the last 12 months

Outcomes

  • What are you as the referrer and the child / young person and their family wanting from a DSR referral.
  • How will we know that we have met the child’s outcomes
