Transition from Early Years to Reception
If your child has been identified as having special educational needs whilst in the early years foundation stage, they may have been offered support from our Early Years SEND Practitioners. The team have worked closely with schools, private nurseries and families to understand the worries that there may be about supporting your child into the next phase. Please see the transition document which highlights the concerns that families raised in a survey.
From this work, the My Transition Plan document was developed. This should be completed together with early years provider, parents and/or any other professional working with the child. The aim of this document is to ensure that the receiving setting has a full picture of your child and what works best for them. Please see below:
